Home School Registration

Utah State Law requires when a parent/legal guardian withdraws an enrolled school-aged child for the purpose of home-schooling or chooses to not enroll a school-aged child, the parent shall provide a one time notification to the local school board of the school-age child’s district of residence. Please complete and submit the online form at homeschool.canyonsdistrict.org.

If you will no longer be home schooling your child(ren); please complete the Home School Non-Renewal form and return it to Canyons Admissions Office or email it to marilee.ostrowski@canyonsdistrict.org.

Exemption Process

  1. A parent must register their student for home school prior to being withdrawn from their public boundary school.
    • Complete the Home School Affidavit.
    • Forms are available in the Student Enrollment office located at 9361 South 300 East, Sandy, UT 84070 (please use south entrance) or Print out form online HERE.
    • Mail or deliver to Canyons School District Office 
    • Once the Home School Affidavit is processed by the Student Enrollment office a copy of the signed certificate of exemption will be mailed back to the parent.
  2. Once Student Enrollment signs the certificate, a copy will be emailed to the school to allow the parent/guardian to withdraw the student(s) from public school.
