This Privacy Policy Statement is provided by Canyons School District in compliance with Utah Code Section 63D-2-103.  

We use the minimal extent of information we need to provide students and parents with the educational services requested. We safeguard your student data and do not improperly share it in accordance with federal and state laws (e.g., FERPA and the Utah Student Privacy and Data Protection Act).

What Information We Collect

We collect and use student and parent/legal guardian personal information for the objectives of fulfilling educational purposes specified by Canyons School District and for other compatible purposes, unless we obtain the consent of the parent/legal guardian and/or individual concerned or as required by law.  

• Student name, date of birth, and sex
• Parent/legal guardian and student contact information and custodial parent/legal guardian information
• A student identification number (including the student’s school ID number and the state‐assigned student identifier, or SSID)
• Local, state, and national assessment results or an exception from taking a local, state, or national assessment
• Courses taken and completed, credits earned, and other transcript information
• Course grades and grade point average
• Grade level and expected graduation date or graduation cohort
• Degree, diploma, credential attainment, and other school information
• Attendance and mobility
• Drop‐out data
• Immunization record or an exception from an immunization record
• Race, ethnicity, or tribal affiliation
• Remediation efforts
• An exception from a vision screening required under Section 53G‐9‐404 or information collected from a vision screening described in Utah Code Section 53G‐9‐404
• Information related to the Utah Registry of Autism and Developmental Disabilities (URADD), described in Utah Code Section 26‐7‐4
• Student injury information
• A disciplinary record created and maintained as described in Utah Code Section 53E‐9‐306
• Juvenile delinquency records
• English language learner status
• Child Find and special education evaluation data related to the initiation of an IEP

Certain sensitive information on students collected via a psychological or psychiatric examination, test, or treatment, or any survey, analysis, or evaluation is collected with parental/legal guardian consent. You will receive a separate consent form in these cases. See our Protection of Pupil Rights Act (PPRA) notice for more information.

Prohibited Collections

We will not collect a student’s Social Security Number or criminal record, except as required by Utah Criminal Code Section 78A‐6‐112(3), except in rare cases with parental/legal guardian consent. This is a run-on sentence.  Did we mean to finish the sentence with the code citation? 

How We Collect and Use Your Information

We will collect personal information by lawful and fair means and, where appropriate, with the knowledge or consent of the parent/legal guardian and/or individual concerned. We will only retain personal information as long as necessary for the fulfillment of educational purposes and state laws.  

Disclosure Practices

We will share personally identifiable information or student data in accordance with the Family Educational Rights and Privacy Act FERPA), which generally requires written parental consent before sharing student data. FERPA includes exceptions to this rule, where we may share student data without parental consent.

You have the right to access and correct your information.  Here’s how you can do it:

Keeping Your Information Secure

We will protect personal information by reasonable security safeguards against loss or theft, as well as unauthorized access, disclosure, copying, use, or modification. We will make readily available to parent/legal guardian and/or student information about our policies and practices relating to the management of personal information.  

We are committed to conducting our business in accordance with these principles in order to ensure that the confidentiality of personal information is protected and maintained.
